Black Infant Health
The Black Infant Health (BIH) program connects pregnant and parenting African and African American individuals with the care they need for good health during pregnancy and beyond. BIH provides eligible individuals with strength-based pregnancy/new parent groups, connection with a personal public health nurse and more. Participants can take part in the program until their child turns 1.
The program is open to individuals who are 16 and older, individuals of African/African American descent, individuals who are currently pregnant, individuals parenting an infant less than 6 months old at the time of enrollment, and individuals living in Santa Clara County.
CalWORKS Home Visiting Program
The CalWORKS Home Visiting Program connects parents—with more than one child—to registered nurses who provide 1-1 parenting support. The program also provides healthcare referrals and social services. Children under 2 will receive monthly home visits until all developmental concerns are resolved.
This program is open to individuals living in Santa Clara County, active CalWORKS participants at time of enrollment, and pregnant individuals or individuals parenting a child under 4.
Early Head Start
Early Head Start home visiting is designed to nurture healthy attachment between a parent and their child (and child and their caregiver). Services include the full range of a family’s needs from pregnancy through a child’s third (3) birthday. Young children receive 46 home visits per year. 90-minute visits are available for pregnant individuals.
This program is open to young children (0 to 3), families living in Santa Clara and San Benito County, families with incomes at or below 100% of the federal poverty level, current CalWORKS participants, Supplemental Security Income (SSI) recipients, foster children, and families experiencing homelessness.
Early Start Program
The Early Start Program (ESP) serves children with disabilities, including developmental delays, hearing impairments, vision impairments, motor impairments, autism, and multiple disabilities. The ESP builds upon and provides support and resources to family members and caregivers to enhance a child development through everyday learning opportunities.
This program is open to young children (0 to 3). Young children with solely low incidence disability including one or more visual, hearing, or orthopedic impairments, and dually eligible children including children with speech impairments, trisomy 21, autism, and other health impairments are also eligible.

KidConnections Network
(Early Childhood Mental Health Outpatient Consortium)
KidConnections Network (KCN) provides developmental and behavioral health services to children and their families. With a focus on the caregiver/child relationship, KCN uses a team approach to support families.
This program is open to families with young children who live in Santa Clara County, young children (0 to 5), and families with children on Medi-Cal.

(FIRST 5 Public Health Nurse Visitation Program)

Nurse-Family Partnership
The Nurse-Family Partnership (NFP) Program empowers clients to transform their lives and create a better future for themselves and their babies. A free personal nurse (or other trained staff) makes weekly or bi-weekly visits to the family’s home to give support for: having a healthier pregnancy, improving a child’s health and development, and more. Individuals can enroll in the program during the first 28 weeks of pregnancy and until their child turns 2.
The program is open to families with low incomes, first-time parents, individuals who are less than 28 weeks pregnant, and individuals living in Santa Clara County.

ParentChild+
Through ParentChild+, an Early Learning Specialist visits a parent/caregiver and their child (16-36 months) at home, at a Family Resource Center, or virtually. Each week, the Specialist provides books and toys to support the child’s early learning and social-emotional development. These services are provided until the child goes to preschool.
This program is open to parents/caregivers with a young child who falls within the intake window (16-36 months) and families with low incomes (or other barriers) and families enrolled in CalWORKS.

Regional Nursing Home Visitation Program
A free personal nurse (or other trained staff) makes regular visits to the client’s home to give nursing and case management support. Participants also receive support to access medical care or to manage a chronic health condition. Case management for children and youth who have or are at risk for lead poisoning is also available.
This program is open to families who are uninsured and underinsured.

Teen Parent Support Program
The Teen Parent Support Program by Planned Parenthood Mar Monte provides free support for pregnant and/or parenting youth. Case managers visit participants throughout Santa Clara County to help youth reach their goals, promote a healthy and nurturing environment for young people and their families, and assist participants with obtaining their high school diploma or GED certificate. There are no income requirements. The program offers two in-person or virtual visits per month.
This program is open to individuals (under the age of 21) who are pregnant or parenting young children and live in Santa Clara County.
